Access to a quiet or reduced-distraction testing space

Advance notice of schedule changes, fire drills, and substitutes

Medication administration plan and nurse-office access

Food-allergy, diabetes, or seizure action plan attached

Unrestricted bathroom and water access when medically needed

Emergency evacuation plan adapted for mobility needs

Confirm every teacher receives the plan in the first week of school

Name one staff member as the accountable point of contact

Schedule a check-in six weeks in

Review annually and update after any diagnosis or placement change
